What is Body Mass Index (BMI) and How is it Calculated?

Body Mass Index (BMI) is a simple numerical screening tool used worldwide to evaluate whether a person has a healthy body weight relative to their height. Developed in the 19th century by Belgian mathematician Adolphe Quetelet, BMI provides an accessible estimate of total body fat categories for adult men and women.

The Mathematical Formulas for BMI

BMI is calculated by dividing body weight by height squared. The exact formula depends on whether you are using metric or imperial measurements:

  • Metric Units:
    BMI = weight (kg) ÷ [height (m)]²
    Example: An adult weighing 70 kg and standing 1.75 meters tall has a BMI of 70 ÷ (1.75 × 1.75) = 22.86.
  • Imperial Units:
    BMI = [weight (lbs) ÷ [height (inches)]²] × 703
    Example: An adult weighing 154 lbs and standing 5 feet 9 inches (69 inches) tall has a BMI of [154 ÷ (69 × 69)] × 703 = 22.74.

The World Health Organization categorizes adult BMI values into four main weight ranges:

Key Limitations of BMI

While BMI is an efficient population-level screening metric, it is not a diagnostic tool. It does not measure body composition directly and can overestimate or underestimate health risks in several key groups:

  • Athletes and Bodybuilders: Muscle tissue is significantly denser than fat. Highly muscular individuals may score in the "Overweight" or "Obese" categories despite having low body fat percentages.
  • Pregnant and Lactating Women: BMI calculations do not isolate fetal weight, placenta, or increased fluid volume from body fat, rendering standard ranges inaccurate during pregnancy.
  • Older Adults (Sarcopenia): As people age, natural loss of muscle mass (sarcopenia) can cause an elderly individual to have a healthy BMI despite having higher proportions of body fat.
  • Ethnic Variances: Risk thresholds vary across populations. For instance, the World Health Organization notes that South Asian and East Asian populations face heightened cardiovascular and diabetic risks at lower BMI thresholds (starting at 23.0 kg/m²).
  • Fat Distribution: BMI cannot differentiate between harmful abdominal (visceral) fat and subcutaneous fat. Two people with identical BMIs may carry vastly different health risks depending on waist circumference.
